SERVICE DELIVERY PLAN

The timeframe to complete a project will be dependent on a number of factors including; overall scope of work and associated complexity, availability of hardware and software, customer scheduling and availability, and User participation. Caresoft will work closely with you to ensure the deliverables of the SOW are completed in a timely and organized manner. Caresoft anticipates most of the planning, development and testing work will occur during standard business hours. When required, due to active production system impacts or other constraints, work will be conducted during customer accepted timeframes as to not affect business operations.

At a high level the plan is divided into the following 7 Phases. Please note that the actual project plan may deviate from this specially phases 4 (Testing) and phase 5 (Cutover/Deployment) as these are very specific to each project and customer environment.

PHASE 1 – PROJECT PREPARATION AND PLANNING TASKS

This includes the project “kickoff” meeting, deployment of the project plan, and addressing other relevant issues necessary to start the project.

PHASE 2 – REQUIREMENTS GATHERING AND SYSTEM DESIGN

During this phase the objectives and requirements of the project are established. Using an object-oriented approach to the development of software architecture, Caresoft will systematically and iteratively progress through the software design process

PHASE 3 – APPLICATION DEVELOPMENT

During this phase all required components of the application are carefully developed, unit tested and implemented to meet customer requirements.

PHASE 4 – VALIDATION

During this phase, Caresoft will help the customer test the application in their environment with the participation of end users.

PHASE 5 – CUTOVER / FINAL DEPLOYMENT

The new application will be activated and put into the production environment. Caresoft will be available to address any related system or end-user issues and questions. Caresoft will assist with post-implementation issues for the duration of this project. Customer is responsible for requesting configuration changes and bringing up any issues to the attention of Caresoft engineers within the project timeframe.

PHASE 6 – KNOWLEDGE TRANSFER / DOCUMENTATION

In general, Caresoft will perform knowledge transfer to the customer’s IT staff throughout the project. Prior to the project closeout meeting, Caresoft will perform a session to review the configuration of the final deployed solution with the customer. Caresoft will deliver project related documents, both in hard-copy and electronic formats. This is to enable the customer to perform ongoing maintenance and enhancement of the system.

PHASE 7 – PROJECT CLOSEOUT MEETING

At this meeting the final project documentation is delivered, the final Deliverables Acceptance Form is presented for approval, and Customer Satisfaction Survey is conducted.
